We are a rapidly expanding team seeking a Clinical Director, Associate to join us in transforming healthcare for seniors. In this role, you will lead a care team, provide direct patient care, and mentor and coach primary care providers in delivering outstanding clinical outcomes.   • Provide care for patients with acute and chronic illnesses
  • Take full accountability for patient care and outcomes
  • Engage with hospitalists and specialists as needed
  • Lead care team and support clinical operations
  • Mentor and coach primary care providers
